These forms will be available at the office, but you may elect to complete them prior to your first visit. This file includes the office policy and the HIPAA privacy notice. Please retain a copy for your records.

Confidentiality
 
The physicians and staff of Hudson Psychiatric Associates, LLC, takes your confidentiality seriously and complies with the Health Insurance Portability and Accountability Act. The privacy of our patients is respected, and, except in a few cases mandated by state and federal law, information about treatement and care will not be released without the patient's express consent.
 
HIPAA Privacy Notice The most recent version of our privacy policy is available online and at our office.
 
HIPAA Authorization for Release of Information Please download and complete as many forms as needed to send to your past or current treatment providers. These are required for us to obtain clinical information helpful for your diagnosis and treatment.
 
Office Policy Please review our policies regarding appointments with your psychiatrist.
 

 

Payment

 

Payment is expected at the time of the service. Checks and credit cards (Mastercard & Visa) are accepted. A receipt will be provided that can be submitted to your health care plan for out-of-network benefit reimbursement. Please note that it is your responsibility to be aware of your benefits including coverage eligbility and need for a deductible.

 

Cancellation

 

Because your appointment time has been reserved for you, you will be charged for cancellations with less than 24 hours (one business day) notice. For example, if your appointment is scheduled on a Monday or following a long weekend, please call on the preceding Friday. Charges for missed appointments are not covered by your insurance and are due and payable prior to any further appointments.

Edinburg Postnatal Depression Scale (EPDS) Reformatted screening form for depression postpartum.

 

Adult ADHD Self-Report Scale (ASRS) is a self-report list that is a useful diagnositc measure for diagnosing ADHD. The latest version, as well as forms in foreign languages, are available here.

 

The Mood Disorders Questionnaire (MDQ) is a 13-item checklist that may be helpful when presented to your psychiatrist or doctor for identifying signs of bipolar disorder.

 

The Patient Health Questionnaire (PHQ 9) contains a brief, 9-item, patient self-report depression assessment specifically developed for use in primary care.
